This dashboard is the governed testing record for ACA v1.5.5 EN-US PRE-BASELINE. Package tests and live-deployment tests are maintained as separate evidence states. Historical results are preserved rather than overwritten.

ACA-CSF-2026-09-09-006 — Post-deployment testing dashboard synchronization / propagation failure
After the CSF-005 synchronization package was deployed, Home, Machine Access, and Kayla’s Thoughts advanced while the live Testing Dashboard continued serving stale RC3-era/current-state content. Package correctness therefore did not propagate uniformly to the live governance surface.
Permanent regression rule: after any deployment that changes testing or failure state, /testing/, /testing/results.json, /feeds/testing-evidence.xml, and /feeds/ai-failures.xml must be purged and force-validated together. Deployment acceptance fails if any one remains stale or if human and machine projections disagree.

ACA-CSF-2026-09-09-005 — Testing Dashboard current-state synchronization failure
Post-deployment scan found the human Testing Dashboard still carrying RC3-era package wording, deprecated /feed/ architecture language, and pre-deployment observations even after RC7 was live. The defect could cause human governance interpretation to diverge from current machine and deployment state.
Permanent regression rule: testing dashboard rows, machine testing ledger, testing RSS, failure register, feed-path descriptions, and release state must be derived or reconciled from the same current governed records before deployment acceptance.
Common-Sense Failure
ACA-CSF-2026-09-09-004 — Global surface contrast / text-color inheritance failure
Live inspection of Kayla’s Thoughts showed light/tan Learning Journey cards rendering with very light text, reducing readability and creating an accessibility risk. The defect is classified as blocking for baseline freeze.
Permanent regression rule: every reusable surface explicitly owns foreground and background colors for relevant descendants; rendered contrast checks are required across major component families.

ACA-CSF-2026-09-09-002 — Global visual-shell inconsistency
Live desktop inspection identified inconsistent navigation typography and oversized research-card headings across governed pages. The same-release correction establishes a single desktop header scale and bounded research-card title scale. Baseline freeze remains blocked until rendered and live validation pass.
